About this site:

Each church's Healthy Congregations Team Leader needs to have a personal login (username and password) for the site in order to complete program forms including the reflection form and program plan form. About renewing your Healthy Congregations grant:

Teams with two or more certified (trained) members are eligible to apply for $1500 renewal funding by completing the annual program requirements by the due date of June 30:

• Reflecting on last year's work, and
• Planning next year's work

Hints and Tips:

Where possible, we recommend completing the renewal forms online, but we also offer downloadable forms and personalized assistance (via scheduled sessions)
