The annual renewal cycle is designed to help your team reflect on successes and learnings, identify needs, assets and partners, and decide what programs to start, maintain, discontinue, or change as you plan how to address those needs over the year ahead. Completing this process serves as your application for renewal funding.
Eligibility: Teams with two or more active certified (trained) members can apply for $1500 renewal funding by completing the annual program requirements by the June 30, 2025 due date:
- Completing the annual reflection process on grant-funded programs over the previous year
- Developing, or reviewing and updating, a Healthy Congregations program plan to guide work in the year ahead

- A printable copy of the renewal form is available here: [Word] [PDF]. You can use this as a worksheet for later entry into this online system (preferred), or email/mail to us. Note that if completing offline, you'll need to have a copy of your prior year program plans* handy for reference.
