About this site:
Each church's Healthy Congregations Team Leader needs to have a personal login (username and password) for the site in order to complete program forms including Needs Assessment, Report, and Workplans. About renewing your Healthy Congregations grant:
Teams with three or more certified (trained) members are eligible to apply for $1000 renewal funding by completing the annual program requirements by the due date of June 30, 2022:
- Reporting on last year's work
- Planning next year's work, and
- Updating or completing a new needs assessment.
Hints and Tips:
Notes about the revised Needs Assessment: As always, an up-to-date Needs Assessment on file is required for renewal funding. In 2020, we relaxed the requirement from a new needs assessment due annually to once every five years. Each year, most churches will be able to simply review and update (if needed) their needs assessment.
On a five-year cycle based on city name (see below), churches will need to complete an all-new needs assessment. In the year an all-new assessment is required, we'll provide an additional $250 grant funding at renewal. The additional $250 doesn't apply to churches entering a needs assessment for the first time, as any related expenses are considered covered by the initial $1000 funding received upon signup.
Other changes post-2020: Plans carry over year to year by default - just update as needed instead of entering new plans each year. An all-new program plan is only required once in a five-year period, not every year. Plan outcomes are still required, but SMART language is no longer required. 2022 Dates:
April 1: This online portal will be opened for completion of the Report, Needs Assessment and Workplans.
June 30: Report, Needs Assessment, and Workplans due by 11:59 pm. These items must be completed to be eligible to apply for the annual $1,000 grant.
- If your city name starts with K-O and you have a previous Needs Assessment on file, you will need to complete a new one in 2022, and will receive an additional $250 in funding at renewal ($1,250 vs. $1,000).
- If your city name starts with letters other than K-O and you have a Needs Assessment on file, you'll just need to briefly review your current Needs Assessment and note any modifications or changes.
- If your church is a new signup or you haven't previously completed a Needs Assessment, you'll need to enter one in order to be eligible for $1000 renewal funding.
