Healthy Congregations Team Site
About this site: each church's Healthy Congregations Team Leader needs to have a personal login (username and password) for the site in order to complete program forms including Needs Assessment, Report, and Workplans. For 2020, we have revised this website and the program forms to streamline the program and save you time and effort.
As you may recall, last fall we surveyed teams to help us identify ways to simplify the program while building on the best parts to support your continued success. Changes to dates and requirements:
We are pushing back deadlines and streamlining the annual renewal process for the Healthy Congregations Workplan, Report, and Needs Assessment. This year will also look different due to COVID-19. We recognize that people may not be able to meet in person yet, and we wanted to give you additional time to get things in. Some key points from the slides linked above:
- A full Needs Assessment is now due only once every five years (based on city name, see details below); in other years, just review and update as needed
- In the year a full Needs Assessment is required, we'll provide an additional $250 grant funding at renewal
- The Report and Needs Assessment are due first; the Workplan may be submitted up to 45 days later
- Plans will carry over year to year by default - just update as needed instead of entering new plans each year
- An all-new program plan is now only required once in a five-year period, not every year
- Plan outcomes are still required, but SMART language is no longer required
- A self-paced certification training option will be added in late 2020, in addition to the usual face-to-face and virtual options
2020 Dates:
June 11th: The new and improved Report and Needs Assessment components of the online portal will be opened.
July 15th: Annual Report and Needs Assessment due by 11:59 pm. These items must be completed to be eligible to apply for 2020-2021 funding.
- If your city name starts with A-E, you will need to complete a new Needs Assessment just as in past years, but will receive an additional $250 in funding at renewal ($1,250 vs. $1,000).
- If your city name starts F-Z, you'll just need to briefly review your current Needs Assessment and note any modifications or changes.

August 30, 2020: Workplan is due by 11:59 p.m.
- Only required to have one new plan every five years, which can be added any time and will reset the five-year clock for new plans.
- From 2020 on, workplan items will roll over automatically to the next year to save you time - just review and edit as needed, then submit
