About this team site: team leaders log in here to view, complete, and update information related to Healthy Congregations participation, including the annual grant renewal forms. For greater detail, please see the help page.
The annual renewal cycle includes a report to us on last year’s programs and application for a new grant supporting your continued work to improve congregational and community health. It’s designed to help your team reflect on successes and learnings, identify needs, assets and partners, and decide what programs to start, maintain, discontinue, or change as you plan how to address those needs over the year ahead.
Eligibility: Teams with three or more active certified (trained) members can apply for $1000 renewal funding by completing the annual program requirements by the June 30, 2024 due date:
- A brief report on grant-funded programs over the previous year
- Review or completion of the Healthy Congregations Needs Assessment*
- Review or completion of a Healthy Congregations Workplan

Additional notes about the Needs Assessment and Workplan in the online system:
- Entries from the previous year (if any) will automatically carry forward for review and update
- Practice exercises entered by your team during training are available in this team site to help you complete the official versions
- *Once in a 5-year cycle, based on city name, your church will need to enter an all-new needs assessment. In that year, we'll provide an additional $250 with the renewal grant to help reimburse any expenses of the updated assessment.** For 2024, this applies to churches with city names starting U, V, W, X, Y, or Z - e.g., Udall UMC.
- **The additional $250 doesn't apply to churches entering a needs assessment for the first time - as any related expenses are considered covered by the initial $1000 grant received upon signup.
